New clients are encouraged to print out the intake forms and bring them to my office at the time of your first appointment.

Plan to arrive about 5-10 minutes early and you can fill them out in our reception room.  I will come out to meet you at your scheduled appointment time.  Please feel free to ask me any questions about our policies.
